Test Description

Prolactin is a hormone produced by the anterior pituitary gland, and it helps to initiate and maintain breast milk production in pregnant and nursing mothers. Prolactin test measures the
level of prolactin in the blood and it can help determine the cause of abnormal milk production in a female who is not pregnant or breastfeeding, diagnose the cause of infertility in couples, and erectile dysfunction in men, it also helps to diagnose the cause of menstrual irregularities in women. Prolactin test can also be requested in evaluating pituitary disorder and tumors which secrete excess prolactin.    

Production of prolactin follows a circadian rhythm and levels are highest in the morning. So blood prolactin test is usually done about 3 – 4 hours after waking.

When To Get Tested

  • If you have signs and symptoms of a prolactin-secreting tumor such as an unexplained headache, visual impairment and unexplained nipple discharge In a woman being evaluated for infertility or irregular menstruation 
  • In a man, who is being evaluated for infertility, also if he has symptoms such as reduced libido (sex drive), nipple discharge or low testosterone level
  • To monitor treatment  or diagnose reoccurrence in a person diagnosed with prolactinoma 
  • If you are suspected to have a pituitary disorder such as hypopituitarism 
  • If you are on medications that can affect dopamine secretion 

Test Result & Interpretation

  • High prolactin levels are normal in pregnancy, and in nursing mother
  • High prolactin levels can indicate various diseases such as kidney disease, liver disease, polycystic ovarian syndrome, pituitary disease and tumors, disease of the hypothalamus, hypothyroidism, and prolactinomas.
  • If prolactin levels are low, it may indicate a pituitary disorder with associated generalised decrease of other pituitary hormones as occurs in hypopituitarism

Common Questions

What other test will I be asked to do to evaluate high levels of prolactin? 
Other tests that can be requested to evaluate prolactin levels include testosterone in males, FSH (follicle stimulating hormone), MRI (magnetic resonance imaging) of the brain which shows pituitary enlargement and also can help locate the tumor.

Why do I have to do a thyroid test, when my prolactin level is elevated?
Most often elevated prolactin levels are seen in people with hypothyroidism, however, it doesn’t cause it.

What is the treatment for prolactinoma?
Prolactinomas can be treated with medication or surgery; medications used are drugs that act like dopamine (which inhibits prolactin secretion) such as bromocriptine or cabergoline. If there is no
response to medication, surgery can be done.

I’ve heard of macroprolactin, can you explain it? 
Macroprolactin is an inactive form of prolactin; it is bound to proteins and is not a cause for concern. However, can cause elevated prolactin levels, so further evaluation is done in people with elevated prolactin to be sure it is not due to macroprolactin.
